Every requisition needs the patient's full name, address and date of birth, plus both a phone number and an email address.

If the patient has coverage, include their insurance details: policy holder, insurance company, group number and subscriber ID.

For imaging taken in our office, choose whether it is billed to the patient or to the referring doctor.
